Which of the following individual findings would be most concerning and prompt further evaluation of the infant in Case 1?
WHY the answer is conjugated bilirubin of 4 mg/dl, i thought is C, unconjugated bilirubin. The passage paragraph 2 only mentioned about the unconjugated bilirubin leads to jaundice.

Good question. A lot of this hinges on knowing what the use of the word physiological in a (somewhat) clinical context means: A physiological condition is perfectly normal, and usually non-threatening. So physiological jaundice due to excess unconjugated hyperbilirubin, as would be the case for option C, is not a cause for concern.

I wouldn't worry about this particular passage too much, the relative paucity of information one is given to work with here inflates the difficulty artificially - after all, we are never told explicitly that excess conjugated bilirubinemia at about 2x the reference range is in any way concerning.
