This is saying that you want to differentiate acute from chronic needs. The passage states in paragraph 4 ("Ghrelin...") that ghrelin levels flucutate in response to both acute and chronic metabolic demands and that i's action is to increase appetite. Increased appetite leads to an increased intake of metabolic energy. All groups were tested for fasting blood levels of ghrelin, so we can discount differences in acute metabolic demands as the source of the difference in ghrelin levels. That must mean that the chronic metabolic demand in obese individuals is lower than that in lean individuals.

This follows exactly the same reasoning. Their chronic metabolic energy demands are changing. They are over-fed while gaining weight, so their ghrelin levels should be low. They are under-fed while losing weight (that is after all how you lose weight), and so their circulating ghrelin levels should be high.
